PHOTO AND MEDIA RELEASE

The undersigned Releasor, (“I”), hereby agrees as follows: 

 

A. I consent and authorize Balanced Birch Studio, LLC (the “Company”), a Massachusetts limited liability company located at 77 Cowls Rd, Amherst, MA, 01002, to use my likeness in any photograph, video or any other digital media (each a “Photo”), at any time taken or to be taken before, during or after our sessions or any other Company events, in any publications, including print or web-based publications, and for all marketing and promotional purposes. 

 

B. I irrevocably authorize the Company to copy, edit, enhance, crop, or otherwise alter any Photo at any time, and I waive any rights for approval or inspection of any Photos. 

 

C. I understand and agree that all Photos and all copyrights therein and thereto are the property of the Company and will not be returned to me.

 

D. I acknowledge that I am not entitled to any compensation or royalties with respect to the use of the Photos.

 

E. I release and forever discharge the Company and its affiliates, successors and assigns, and their respective managers, members, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities (each, a “Releasee”), from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Releasee in connection with this Photo and Media Release.

 

F. This Photo and Media Release shall be binding upon my heirs and legal representatives and shall inure to the benefit of the Company, its successors and assigns.

G. I have carefully read and fully understand all the provisions of this Photo and Media Release and am freely, knowingly and voluntarily signing it.
